How the score is calculated
- Bilirubin, creatinine and INR below 1.0 are set to 1.0.
- Creatinine above 4.0 mg/dL (354 µmol/L) is set to 4.0 mg/dL. With dialysis as above, creatinine is set to 4.0 mg/dL.
- Sodium is bounded between 125 and 137 mmol/L.
- MELD = 10 × (0.957 × ln creatinine + 0.378 × ln bilirubin + 1.120 × ln INR + 0.643), rounded to the nearest whole number. Laboratory values are in mg/dL.
- If MELD is above 11: MELD-Na = MELD + 1.32 × (137 − Na) − 0.033 × MELD × (137 − Na).
- The final score is rounded to a whole number and capped at 40.
Conversions: bilirubin µmol/L ÷ 17.1 = mg/dL; creatinine µmol/L ÷ 88.4 = mg/dL.
